Information you submit

Report-interest, correction, and contact forms may collect your name, email address, facility reference, public-source links, and message. Do not send medical records, government identifiers, payment-card data, or other sensitive personal information.

Technical information

The hosting platform may process ordinary security and access logs such as IP address, browser type, requested page, and timestamp. WordPress may use essential cookies for administrators and form security. Third-party analytics and advertising trackers are not enabled by the CareLedger build by default.

How information is used

Submitted information is used to respond to requests, review sources and corrections, prevent abuse, and improve the directory. Report-interest forms do not collect payment information or create a charge.

Your choices

Use the Contact page to request access to or deletion of information you submitted, subject to legal, security, and record-integrity requirements. Public facility facts come from named government sources and should be corrected through the documented correction process.
